Complete your registration.
It takes a few minutes. Have the participant’s NDIS number, date of birth and NDIS plan document handy. Don’t have the plan? Submit what you can now and email it to hello@zebracare.com.au later.
Then, endorse Zebra Care with the NDIA.
This is the step that makes us your Plan Manager, and it only applies to PACE plans. Not sure if you’re on PACE? Complete the registration above and we’ll check for you.
Call the NDIA on 1800 800 110 and ask them to endorse Zebra Care as your Plan Manager. They’ll ask for:
